Health Promotion Lead

 

Home Based 

 

38 hours per week 

 

£37,370pa with £4,224pa for Car Allowance  

 

Purpose of Role

 

Ambient support people with many, varying and often complex health needs and without good health and a sense of wellbeing, people are not able to engage with the support we can offer. We will be recruiting a Health Promotion Lead, which will focus on:

 

*       Improving the health and wellbeing of the people we support

*       Enhancing the quality of life for people

*       Reduce health inequalities for the people we support.

*       Health education for the people we support, and our colleagues to provide better health outcomes for people

*       Empowering people to take control of their own health and wellbeing.

*       Preventive work across Ambient’s portfolios

*       Promote Health initiatives within Ambient & ensure we are following evidence-based practice.

 

Health Promotion

 

*       Lead on the reduce of health inequalities for people who Ambient support. Identify, challenge and remove existing and potential barriers to people accessing health and wellbeing services.

*       Provide health education to people we support and to teams on key health concerns, such as constipation, Epilepsy, Falls Prevention, Diabetes etc

*       Ensure Ambient are working within any relevant best & evidence-based practise to improve the lives of people who receive support from Ambient.

*       Deliver specific health promotion/education sessions to people we support, families and Ambient staff.

*       Review best practise guidance, horizon scan to ensure Ambient is working in the most current & evidence-based approaches to improve the health & wellbeing of people.

*       To improve practise in our homes & services to ensure people receive the best standard of care to improve their health & wellbeing.

*       To review the annual LeDeR report and any other health and wellbeing reporting and review against Ambient’s policy & practise, with a view of making any necessary improvements.

*       Identify, and support with the roll out of any health specific diagnosis training, support & coaching to operational teams.

*       To work in partnership and develop links with local community groups, voluntary organisations, and statutory bodies.

*       Be the IPC Lead for Ambient.

 
 

*       To monitor & review internal data such as Falls, medication errors, ABC’s, serious incidents, health related safeguarding concerns etc, to identify, themes & trends to drive organisational or service specific improvements. 

 
 

Health Promotion Lead & Quality  

 

*       To support staff and teams regarding the risk management of people we support’s health and wellbeing.

*       To develop and deliver learning opportunities relating to best practise service provision.

*       To lead on identifying, sharing, and implementing evidence-based practise through quality improvement and service development.

*       Monitor the use of forensic & other clinical health risk registers within Ambient and support teams to ensure that people receive safe care, & the appropriate MDT & external health & social care colleagues are involved.

*       Provide advice and guidance to teams on practise matters affecting good quality service delivery.

*       Support initiatives and focus on medication error reduction across Ambient.

*       To ensure Ambient have the appropriate health records for monitoring & capturing the support people need to improve their health and wellbeing.

*       Deliver initiatives on improving the wellbeing of people we support (relationships, mental health, exercise, healthy eating etc)

Quality and Service Improvement 

*       To work with Ambient Guardians to ensure that we work collaboratively and people we support are involved in organisational change.

*       To provide targeted support and guidance to services that require an extra focus to drive quality improvements.

*       To report on service / people and or organisational compliance or reputational risks and to be solution focused in supporting any necessary service or organisational improvement.

*       To shape audit tools and governance tools in measuring quality of support and the quality of life of people we support

*       To identify opportunities for quality initiatives, leading on the delivery of those in services and/or organisationally

*       Identify any themes & or trends in any quality or practise data (qualitative or quantitative) and from this identify solutions to ensure we continuously improve Ambient service deliver and governance.

*       Where necessary to carry out practise observations, audits and reviews of services and implement changes that may be required from findings.

 
 

Qualifications/Experience:

 

*       Relevant Health Promotion Qualification (or health related qualification)

*       Coaching and mentoring teams

*       Experience in delivering health promotion sessions to diverse stakeholders.

*       Experience of working with diverse stakeholders

*       Experience in driving quality improvements.

*       Experience of working in the charitable sector, educated to degree or equivalent would be desirable.

 
 

Essential Skills/Knowledge/Attributes:

 

*       Excellent knowledge of health promotion with people with a Learning Disability, Autism, Older Adults & people with a Mental Health Diagnosis 

*       Good knowledge and use of Teams/Microsoft

*       Computer literate to a high standard

*       A knowledge of the adult social care sector and the legislation that governs it

*       Ability to communicate and work across departments to improve the quality across Ambient.

*       Solution focused and work using initiative.

*       Ability to review themes and trends in data & practise observations.

*       Extensive knowledge of good practise in the Learning Disability, Mental Health & Older Adults Sectors

*       Knowledge of local/national priorities/initiatives/innovations that may impact on policy & practice.

*       Commitment to promoting good practise and quality assurance.

*       Flexibility in approach

*       Willingness to learn and undertake training.
